question | bleeding

Question time again!

A 53 year old gentleman is referred in by his GP for investigation of iron deficiency anaemia. A gastroscopy is performed and this bleeding lesion is noted. 

Any ideas?

Okay, few more clues:

– he suffers from recurrent epistaxis
– he has a family member with similar problems
